A group of five second-year medical students are undertaking a group project which forms part of their grade. One of the group members Alex has turned up 20 minutes late to their third team meeting. He has been late to every single meeting. Sarah, the team leader is becoming slightly frustrated as Alex’s lack of punctuality is disrupting the group’s progress.
How appropriate are each of the following responses by Sarah in this situation?
Explanation
The correct answer is D.
By reporting Alex to their tutor before trying to address the issue, Sarah is unnecessarily escalating the situation. Remember, you should always try and address the issue before escalating it further. Therefore, this action would be very inappropriate.
Top Tip: When you have concerns about a colleague, you should always try and address them directly first before escalating it to someone more senior.

How appropriate are each of the following responses by Sarah in this situation?
Explanation
The correct answer is A.
This is the ideal course of action as it allows Sarah to address the issue of Alex’s punctuality whilst being respectful to him. This is in line with GMC guidelines which states “Persistent inappropriate attitude or behaviour [for medical student includes] poor time management [and] non-attendance]” (Achieving Good Medical Practice: Guidance for Medical Students, p48) and “You must treat colleagues fairly and with respect” (Good Medical Practice, p14). Therefore, this action would be very appropriate.

He has been late to every single meeting. Sarah, the team leader is becoming slightly frustrated as Alex’s lack of punctuality is disrupting the group’s progress.
How appropriate are each of the following responses by Sarah in this situation?
Explanation
The correct answer is C.
This action does not address the issue of Alex’s punctuality. This violates GMC guidance which states “It can be difficult to raise concerns about fellow students, who may be people you work with on projects or placements or your friends. But as a student choosing to join a regulated profession, it is your duty to put patients first and this includes patients you see on placements and those treated by your fellow students in the future.” (Achieving Good Medical Practice: Guidance for Medical Students, p21). However, you are not unnecessarily escalating the situation or doing nothing, so this action is not very inappropriate. Although, since it does not address the issue, this action is inappropriate but not awful.

How appropriate are each of the following responses by Sarah in this situation?
Explanation
The correct answer is B.
This action is appropriate as it allows you to seek advice from a peer. However, you are disclosing this issue to another member of the group unnecessarily. Therefore, this action would be appropriate but not ideal.

true

Explanation
The correct answer is C.
This action would help address the issue by reminding Alex to turn up to the meetings on time. However, this is not addressing any underlying issues Alex may have which is being disrespectful to him. This violates Good Medical Practice which states “You must treat colleagues fairly and with respect” (Good Medical Practice, p14). Therefore, this action is inappropriate but not awful.

group message indicates that its being sent to the whole group not him directly and context doesnt say it was directed at him regardless of what conclusions you can infer from it. why would it be inappropraite?

Tue, 26 Jul 2022 15:10:49
This is to f- Just think about it in a realistic way Alex is the only one always turning up late a message like that sent to the group would only highlight and target him which could make him feel attacked by it and offended. How would you feel if someone sent a message into a group you were in targeting you even though they could've just sent it to you personally

Wed, 03 Aug 2022 02:31:30
@f I guess that's because the text message would come across as pretty vague and not directed at the main issue. Plus even if she's trying to send across a msg to Alex indirectly this way, he probably won't understand. Additionally there could be an underlying reason for his lateness (e.g. some personal issues), so being the team leader sarah should approach Alex directly and politely. I hope that makes sense
James and Kim are two 4th year medical students currently on their hepatology placement. James is making TikToks on the wards, sometimes in the view of patients. Some of the TikToks are making fun of patients talking about alcohol use. Kim can see that some of the patients are looking uncomfortable and she feels that James is acting in an unprofessional manner.
How important is it to take into account are the following considerations for Kim when deciding how to respond to the situation?
Explanation
The correct answer is A.
Patient safety, comfort and dignity must be your top priority. This is outlined in Good Medical Practice which states “You must take prompt action if you think that patient safety, dignity or comfort is or may be seriously compromised” (Good Medical Practice, p11). Therefore, if patients are feeling uncomfortable, you must act. Therefore, this consideration is very important.
Top Tip: Remember, your number one priority MUST ALWAYS be patient safety, comfort and dignity.

another reason why tik toks gotta be banned

The correct answer is D.
Remember patient safety, comfort and dignity maybe your top priority. This is outlined in Good Medical Practice which states “You must take prompt action if you think that patient safety, dignity or comfort is or may be seriously compromised” (Good Medical Practice, p11). Therefore, any impact on yours’s or someone else’s career must not be considered. Therefore, this consideration is not important at all.
Top Tip: Remember, your number one priority MUST ALWAYS be patient safety, comfort and dignity.

Good Explanation, remeber to read up on the GMP guidlines as it helps to clarfiy distinctions between what you think is right and what is actually right

Explanation
The correct answer is B.
It is important to express concerns that a colleague is being unprofessional. This is outlined in Good Medical Practice which states “If you have concerns that a colleague may not be fit to practise and maybe putting patients at risk, you must ask for advice from a colleague, your defence body or us” (Good Medical Practice, p11). However, professionalism may be subjective and so this factor would not be considered as important as patient safety. Therefore, this consideration is important.
Top Tip: Remember, your number one priority MUST ALWAYS be patient safety, comfort and dignity.

I still don't get this! what are they trying to insinuate in the answer.

This is outlined in Good Medical Practice which states “You must take prompt action if you think that patient safety, dignity or comfort is or may be seriously compromised” (Good Medical Practice, p11). The reason, this is just important and not very important is because regardless of whether James is making TikToks about patients on the ward or off the ward, it is still unprofessional. Therefore, this consideration is very important.

I do not understand why this is not very important?

Hi Rosie, thanks for your comment! In this case, it's not very important because the distinction of the TikToks being on the ward is, in relative terms, insignificant. James would be bringing the profession into disrepute whether he was making fun of patients on social media on or off the ward.

Sun, 18 Jul 2021 13:26:36
I was wondering if the fact the tiktoks were on the ward would make it more important as it could be considered a breach of confidentiality?

Sun, 01 Aug 2021 19:22:46
I regarded this as very important, as the most worrying this is not just him making patients uncomfortable, but the massive risk of a breach of confidentiality if any patient or patient information happens to be in any videos.

Wed, 08 Sep 2021 22:19:07
breaching confidentiality isn't very important? This must be incorrect.

Wed, 22 Sep 2021 13:57:26
Thats why tiktoks are bad

Mon, 27 Sep 2021 08:47:35
this should be very important

I marked A: Very Important for this question and got it correct :)
Jacob is a 1st-year medical student, currently living in student accommodation. Following, his exams, Jacob decided to host a party. At around 2 am, the police are called due to noise complaints. When the police arrive, they find a small quantity of cannabis and ketamine in his room and Jacob is arrested for procession of a controlled substance. The next morning, Jacob is charged for drug procession and released. He is unsure whether he needs to inform the medical school.
How appropriate are each of the following responses by Jacob in this situation?
Explanation
The correct answer is D.
It is your professional responsibility to inform the medical school or the GMC if you are charged with or convicted of a crime. This is outlined in Good Medical Practice which states “You must tell us without delay if, anywhere in the world: you have been charged with or found guilty of a criminal offence” (Good Medical Practice, p23). Therefore by failing to inform the medical school, this action would be very inappropriate.
Top Tip: Remember, actions are not taken in isolation. Multiple actions can be very appropriate as you are not ranking options.

This is outlined in Good Medical Practice which states “You must tell us without delay if, anywhere in the world: you have been charged with or found guilty of a criminal offence” (Good Medical Practice, p23). While the medical school is not the GMC, it would be highly inappropriate to not inform them because you are expected to act with probity. Doctors frequently get in hot water with the GMC for not reporting charges or arrests, so it is very important that this is done well!

How appropriate are each of the following responses by Jacob in this situation?
Explanation
The correct answer is A.
By seeking advice from your medical defence organisation, it would enable you to get legal advice and ensure you are represented at any fitness to practice hearing. This is contained within GMC guidelines which state “If you have any questions about what you should declare to the GMC or to your medical school, you can get advice from your medical school, a medical defence organisation or the British Medical Association (BMA)” (Achieving Good Medical Practice: Guidance for Medical Students, p46). Therefore this action would be very appropriate. Remember, these options are not done in isolation, so it would still be possible to seek advice and inform the medical school at the same time.

but you have a legal obligation to inform the medical school promptly

so would this not be delaying communicating this to the medical school and therefore the GMC

Therefore, by waiting until the next semester, you are not informing them ‘without delay’, so this action violates Good Medical Practice. However, as you are still informing the medical school, this is not very inappropriate. Therefore, this action is inappropriate but not awful.

but he isnt responding without delay, so how is this not very inappropriate it is the same as informing them only if found guilty

Sun, 09 Jan 2022 05:32:40
But what if the med school semester starts in like 6 months? You will be convicted also in 3-4 months. So in effect, only informing if found guilty would also be "inappropriate but not awful". I agree with anonymous.
Jonathan, a fourth-year medical student, is having a drink with his tutorial group from medical school in a local pub. They are joking about things they have seen on placements including making some offensive jokes about some of the doctors and nurse they are working with. Jonathan is feeling uncomfortable about this but is unsure what to do.
How important is it to take into account are the following considerations for Jonathan when deciding how to respond to the situation?
Explanation
The correct answer is A.
If members of the public overhead jokes about colleagues and/or things that happened on placement, this could damage trust in the medical profession. Trust in the medical profession is a key principle of Good Medical Practice which states “To maintain your licence to practise, you must demonstrate, through the revalidation process, that you work in line with the principles and values set out in this guidance. Only serious or persistent failure to follow our guidance that poses a risk to patient safety or public trust in doctors will put your registration at risk” (Good Medical Practice, p05). Therefore, this consideration is very important.
Top Tip: Even your actions in your private life must still be professional. If your actions in your private life are unprofessional, this can still give rise to fitness to practice procedure. Therefore, these factors should still be considered as important.

Tue, 27 Sep 2022 11:57:45
Considering patient confidentiality, I would assume that this would be very important. You should never discuss patients.

Explanation
The correct answer is B.
By making jokes about their colleagues, the students are not showing respect for their colleagues. This is outlined in Good Medical Practice which states “You must treat colleagues fairly and with respect” (Good Medical Practice, p14). The reason, this is just important and not very important is regardless of the jokes are about their colleagues or just placement in general, it is still unprofessional. Therefore, this consideration is important.

I thought it would be Minor Importance because it doesnt matter if they are making fun of a college, patient or a random person. In any situation they should not be making fun of anyone, regardless of their standing.

Thu, 16 Sep 2021 22:36:10
Thought it would be not important at all as he should respond regardless of who the issues are about

Regardless of whether their colleagues can hear the jokes, the students are still not showing respect for their colleagues which violates Good Medical Practice which states “You must treat colleagues fairly and with respect” (Good Medical Practice, p14). Therefore, this consideration is not important at all.

Talking about placement is not necessarily a problem as long as the students are being respectful and not breaching patient confidentiality. However, as they might be breaching confidentiality, this factor could be considered but does not have to be. Therefore, this consideration is of minor importance.

Wed, 11 Aug 2021 07:57:48
I think it should be (A) "very important". Although it's ok to talk about some things during the placement, however, in this situation they should not talk about it, because the possibility of breaching the patient's confidentiality is very likely to occur. Thanks!

Mon, 06 Sep 2021 14:09:33
I agree with the comment above, they may be allowed to talk about placements but not in a local pub where members of the public can overhear?

Wed, 08 Sep 2021 22:21:54
We are meant to assume that they breach patient confidentiality because they are having a drink and with the slip of a tongue they can release patient information by mistake.

Sat, 02 Apr 2022 05:34:39
I agree, the main concern here is the breaching of confidentiality. So it is very important for Jonathan to consider this.

Wed, 08 Jun 2022 03:26:26
I thought this would be very important, due to them being in a pub and there is also a chance that a patient information could be spoken about by mistake.

do we get partial marks if we choose answer closest to the correct answer

Thu, 23 Jun 2022 10:32:07
but this is breaking confidentiality, one of the most important things to consider

Fri, 23 Dec 2022 08:58:32
I don't understand how this isn't (A)?

Sun, 14 May 2023 15:18:58
Really? I see stupid patients. I see dumb patients. I see stupid colleagues. Sure it is of minor importance cause I did not break patient confidentiality. *facepalm

I thought what happens in placement is strictly confidential and you must not talk about it after?

Confidentiality is a key principle of medical ethics, this is outlined in Good Medical Practice which states “You must treat information about patients as confidential. This includes after a patient has died.” (Good Medical Practice, p17). Therefore, this consideration is very important.

Olivia is a first-year medical student. She has to complete her ePortfolio by the 30th March. However, she has been very busy and has not yet completed her ePortfolio and only has a week until the deadline. She understands it is her professional responsibility to complete the ePortfolio and it is required for her progression to second year. She is very stressed and is unsure what to do.
Choose both the one most appropriate action and the one least appropriate action that Olivia should take in response to this situation.
Explanation
Most Appropriate – Email her tutor and explain the situation and ask for advice
Least Appropriate – Miss the ePortfolio deadline
Option 3 is the most appropriate option. You have a professional obligation to complete all aspects of your medical school course. This is outlined in GMC guidelines which state “You must engage fully with your medical course by attending educational activities, including lectures, seminars and placements, and by completing coursework” (Achieving Good Medical Practice: Guidance for Medical Students, p9). Therefore, if you cannot complete any coursework on time, you should seek advice from your tutor. This makes option 3 the most appropriate option.
Option 1 is the least appropriate option. You have a professional obligation to complete all aspects of your medical school course. This is outlined in GMC guidelines which states “You must engage fully with your medical course by attending educational activities, including lectures, seminars and placements, and by completing coursework” (Achieving Good Medical Practice: Guidance for Medical Students, p9). This option would violate GMC guidelines and shows a lack of professionalism. This makes option 1 the least appropriate option.
Option 2 is neither the most appropriate nor the least appropriate option. This option still breaches GMC guidelines which state “You must engage fully with your medical course by attending educational activities, including lectures, seminars and placements, and by completing coursework” (Achieving Good Medical Practice: Guidance for Medical Students, p9). However, by informing your tutor, you are taking some steps to remedy the situation. Therefore, this is neither the most appropriate nor the least appropriate option.
Top Tip: You have a professional responsibility to engage in all medical school teaching. If you cannot, you should seek advice from tutors or advisors.

WHY NOT INAPPROPRIATE BUT NOT AWFUL. Sarah isn't doing something very wrong. She is just trying to get to the root of the problem of Alex's poor attendance and if that involves in her reporting him to the tutor, she as a group leader, should be able to do that.
Tue, 03 Aug 2021 11:10:29
To Js comment, it's because they haven't discussed anything with him. There might be a problem he is facing that might need help
Wed, 15 Sep 2021 11:14:05
this should be appropriate but not ideal. it is always appropriate to report unprofessional and problematic behaviour (in this case Alex's repeated lateness) to a tutor. however in this case, it is not ideal as the problem should be addressed with Alex individually, before it is escalated to involve a tutor.
Sat, 18 Sep 2021 22:01:34
i think this should be inappropriate but not awful
Fri, 11 Feb 2022 00:25:47
Should be C as it does no direct/serious harm nor break any protocols
Thu, 23 Jun 2022 12:17:05
hes naughty
Fri, 24 Jun 2022 06:55:53
would this not be C as they are still dealing with the problem and sorting out the disruption to the groups progress but is not addressing the situation locally, without involving more senior professionals which is not ideal but the problem is being dealt with
Wed, 13 Jul 2022 13:05:28
how at all is this completely inappropriate? should be c
Sat, 28 Jan 2023 15:40:28
I thought you are meant to mark the individual response regardless of time or other actions in the scenario... This answer is assuming that nothing has been done beforehand. Reporting unprofessional behaviour should surely always be an appropriate response?
